<p>The <a class="zem_slink" title="Iowa Department of Natural Resources" rel="homepage" href="http://www.iowadnr.gov/">Iowa Department of Natural Resources</a> will be releasing the weekly fishing report on <strong>Thursdays</strong> in an effort to provide the latest information heading into the weekend. The weekly fishing report is compiled from information gathered from local bait shops, angler creel surveys, and county and state parks staff.</p>

<h2>WHAT IS THE <em>Lake Delton Fisheries Restoration Project</em>?</h2>
<p>On June 9, 2008, at approximately, 10:00 AM Lake Delton's weakest link broke.  Six hundred million gallons of water, covering 267 acres, carved its own return path to the Wisconsin River, emptying the lake in just under 45 minutes.</p>
<p>Four homes, a 200 feet wide section of Highway A, the livelihood of dozens of area businesses, the hopes and dreams of every home owner on the lake and ... the fish.</p>
<p>Yes, the fish.   <em><strong>No water. No lake. No fish.</strong></em></p>
<p>All levels vowed to do what they could to restore Lake Delton, but no one knew how they were going to do this.</p>
<p>No budget existed to deal with such an unlikely natural disaster.  Everyone knew something had to be done.  Just WHAT ... and more importantly - HOW - was the real mystery to be solved.</p>
<p>Ben Hobbins, CEO of <a title="website-&#62;IronClads-Lake Resources Group" href="http://www.lakeresourcesgroup.com/" target="_blank"><em>IronClads - Lake Resources Group</em></a> had been thinking about this problem and wondering what he could possibly do to help. Ben came up with an idea and made a call. Ben called his friend Dan Small, TV/Radio show host and nationally recognized outdoor writer, to ask him to join in developing the <em><strong>idea </strong></em>Ben had for Restoring the Lake Delton Fishery.  Dan immediately jumped at the opportunity and the genesis of the <em>Lake Delton Fisheries Restoration Project</em> was begun.</p>
<p>Ben and Dan hosted a press conference [ <a title="website-&#62; Press Conference, Restore Lake Delton Fisheries Project" href="http://www.channel3000.com/news/17329045/detail.html" target="_blank">Online write-up</a> ] [<a title="TV coverage report" href="http://www.c3ktogo.com/news-video/?mgid=18149" target="_blank"> TV video report</a> ] on Thursday 28 August, 2008, at the site of the breach on Lake Delton, with the greening lake bottom looming in the distance.</p>
<p>Repairs to the dam are underway.  The cleaning of the lake and related expenses are likely to be met, just barely.  There will exist not a single penny for restocking of the Lake Delton fishery.  There are just no monies for restocking.  If it happens, the money will need to come from somewhere else.</p>
<p>This is where the <em>Lake Delton Fisheries Restoration Project</em> comes to play.  Ben's  idea is <em>to raise the necessary capital investment to rebuild the fishery through donations by the local public, sporting clubs,  organizations, businesses and corporate donors</em>.</p>
<p>The campaign will rest solely upon the generosity of the fishing and recreation public.  Those who want to continue the outdoor recreation heritage, of which fishing played such a large part, of Lake Delton, Wisconsin.</p>
<p>The RLDF believes help will come from where ever the <em>Love for Wisconsin lives</em>. Local, state, national and international.</p>
<p>We may never fish the waters of the great state of Wisconsin, but we see the value of being a part of keeping the heritage of fishing alive. Our action today, keep the heritage alive for future generations.</p>

Ben had an idea and made a call.</p>
<p>That call went to to his friend <strong>Dan Small,</strong> the nationally honored outdoor communicator who hosts MPTV's <a title="website-&#62;MPTV Outdoor Wisconsin" href="http://www.mptv.org/outdoor_wisconsin/index.php" target="_blank"><em>Outdoor Wisconsin</em></a> and the <a title="website-&#62;Dan Small Outdoors Radio show - Lake-Link.com/radio" href="http://www.lake-link.com/radio" target="_blank"><em>Dan Small Outdoors Radio</em> </a> show.  Ben asked Dan to join in developing the <em><strong>idea </strong></em>Ben had for restoring the Lake Delton fishery.</p>
<p>Dan, knowing the importance of the fishery to the local community and to Wisconsin, enthusiastically jumped on board and the genesis of the <em>Restore Lake Delton Fisheries Project</em> was begun.</p>
<p>These two men started the ball rolling.  But a number of others, public, corporate and private individuals are also involved.</p>
<p>Then there's YOU!  Without YOU, the outdoor enthusiast, recreationalist, this project could not succeed.  We thank you and applaud you for your help in this effort.</p>

<h2>WHY SHOULD I GET INVOLVED?</h2>
<p>1. Have a hand in lake restoration history: Lake Delton is the first lake in the U.S. to be fully restored following a natural disaster.</p>

<content:encoded><![CDATA[<p><span style="font-family:sans-serif,verdana,arial;font-size:x-small;"><span style="font-family:sans-serif,verdana,arial;font-size:x-small;">The Maryland Department of Natural Resources awarded 37 volunteer fire departments located in 12 counties with a 2008 Volunteer Fire Assistance (VFA) grant to increase their capacity for wildland fire suppression. Projects include wildland personal protective equipment, skid units for building brush trucks, replacing fire hoses, upgrading communications equipment, installing dry fire hydrants, and other supplies equipment needed to supress wildfires. </span></span></p>
<p><span style="font-family:sans-serif,verdana,arial;font-size:x-small;"><span style="font-family:sans-serif,verdana,arial;font-size:x-small;">“Our local fire department volunteers are true heroes of their communities, and we are thankful for their steadfast dedication to helping their neighbors,“ said Governor Martin O’Malley. “These grants ensure that these local heroes can continue to safely protect their neighbors and local businesses from wildfires.” </span></span></p>
<p><span style="font-family:sans-serif,verdana,arial;font-size:x-small;"><span style="font-family:sans-serif,verdana,arial;font-size:x-small;">Grant funds received by DNR from the USDA Forest Service funded 37 applications for $90,112 in match funding to fulfill local wildlfire suppression and preparedness needs totaling $286,039 this year. The maximum grant award was $3,000 per department. Grant applications were very competitive and funding was only available to assist 61 percent of the volunteer fire departments that applied this year. Applications are ranked based on project description, local financial support, population, protection area, and other factors. </span></span></p>
<p><span style="font-family:sans-serif,verdana,arial;font-size:x-small;"><span style="font-family:sans-serif,verdana,arial;font-size:x-small;">“We are thankful for the fire protection provided by local volunteer fire departments,” said Monte Mitchell, DNR State Fire Supervisor. “The grants help rural fire departments increase their effectiveness and safety in combating wildland fires in Maryland.” </span></span></p>

<title><![CDATA[DNR and Mirant Continue Partnership to Restore Atlantic Sturgeon]]></title>

<content:encoded><![CDATA[<p><span style="font-family:sans-serif,verdana,arial;font-size:x-small;"><span style="font-family:sans-serif,verdana,arial;font-size:x-small;"> The Maryland Department of Natural Resources welcomes Mirant Potomac River Generating Station’s continued partnership for an Atlantic sturgeon restoration project for the Potomac River. As part of the renewed partnership, Mirant will provide up to $325,000 over the next five years to support restoration activities of Atlantic sturgeon (<em>Acipenser oxyrinchus</em>), the only large bottom feeders present in the Chesapeake Bay. </span></span></p>
<p><span style="font-family:sans-serif,verdana,arial;font-size:x-small;"><span style="font-family:sans-serif,verdana,arial;font-size:x-small;">“Atlantic sturgeon existed historically along the entire Atlantic coast and at one time, played an important ecological role in the Chesapeake Bay ecosystem,” said DNR Secretary John Griffin. “Mirant’s funding and continued support of this fishery restoration project offers hope that the once abundant sturgeon can grow and prosper in its natural habitat again in the future.” </span></span></p>
<p><span style="font-family:sans-serif,verdana,arial;font-size:x-small;"><span style="font-family:sans-serif,verdana,arial;font-size:x-small;">Overfishing, degraded water quality and man-made stream blockages drastically reduced the Atlantic sturgeon’s population. By 1928, sturgeon were rarely caught north of the Potomac River. Research by the USFWS-Maryland Fishery Resources Office concluded in 2004 that Atlantic sturgeon restoration in Maryland would not likely occur without hatchery supplementation. </span></span></p>
<p><span style="font-family:sans-serif,verdana,arial;font-size:x-small;"><span style="font-family:sans-serif,verdana,arial;font-size:x-small;">“We are so proud of our successful partnership with DNR, and we believe wholeheartedly that these mutually beneficial partnerships are an important component of all Maryland’s fish restoration efforts,” said Misty Allen, External Affairs Manager at Mirant Mid-Atlantic LLC. “We recently expressed our steadfast commitment to the project for the next five years to continue sturgeon restoration.” </span></span></p>
<p><span style="font-family:sans-serif,verdana,arial;font-size:x-small;"><span style="font-family:sans-serif,verdana,arial;font-size:x-small;">DNR and Mirant first began a pilot culture trial on the Potomac River in the summer of 2006. Positive findings from the effort led to larger scale culture trial in 2007. Mirant Mid-Atlantic LLC provided funding for manpower, materials, culture space and water supply, while DNR designed and constructed the research facility. </span></span></p>
<p><span style="font-family:sans-serif,verdana,arial;font-size:x-small;"><span style="font-family:sans-serif,verdana,arial;font-size:x-small;">“The overarching aim of our restoration project is to culture a healthy, captive brood stock of genetically diverse sturgeon adults that will provide larvae and juvenile fish for restoration stocking,” explained Brian Richardson, DNR Fisheries Restoration Manager. </span></span></p>

The title track on this EP, <strong>'DNR'</strong>, sounds a little bit like <strong>Editors</strong> with more emotion and a catchy tune, the lack of discernible chorus almost refreshing, offering a welcome contrast to all the predictable <strong>Razorlight</strong>-a-like songs in the charts.<br />

<strong>'Sunken'</strong> is what <strong>The Bravery'</strong> should have been, nailing the carefree spirit with another mesmerising tune. It's a real joy to hear, with a beautiful bassline and spiky guitars. The best is definitely left until last though, <strong>'In The Fall'</strong> being the most urgent and frantic song on the record. It's too short, but boy is it a blast - taking the best elements of the past two tracks, mashing them together and seeing what happens.</p>

<p class="MsoNormal"><span style="font-size:x-small;font-family:Arial;"><span> “Excessive nutrient pollution in the water fuels these algal blooms, which are not uncommon in the freshwater and low salinity waters of the </span></span><span style="font-size:x-small;"><span>Potomac River</span></span><span style="font-size:x-small;"><span>.  Harmful algal blooms such as these can be prevented if more people living in the </span></span><span style="font-size:x-small;"><span>Potomac River</span></span><span style="font-size:x-small;"><span> watershed take steps to reduce run-off pollution,” said Tom Parham, DNR </span></span><span style="font-size:x-small;"><span>DNR</span></span><span style="font-size:x-small;"><span> </span></span><span style="font-size:x-small;"><span>DNR</span></span><span style="font-size:x-small;"><span> assessment director.  “Some easy and affordable backyard solutions include planting native trees and rain gardens, using rain barrels, and refraining from fertilizing in the spring.” </span></span></p>
<p class="MsoNormal"><span style="font-size:x-small;font-family:Arial;"><span>After learning of algal toxin levels higher than typical acceptable standards for human and animal contact in water samples from Mattawoman Creek, <a href="http://mail.atlanticbb.net/Redirect/www.charlescountyhealth.org/" target="_blank">Charles County</a> issued a health advisory for the area encouraging people to take precautions to reduce the risk of illness or discomfort related to blue-green algal blooms:</span></span></p>

<content:encoded><![CDATA[<p>I hadn't thought about this before today - however I had an ephinay while reading <a href="http://www.wausaudailyherald.com/apps/pbcs.dll/article?AID=/20080801/WDH0101/808010637/1981/WDHbusiness">an article</a> that was dripping with the usual nonsense in the Wausau Daily Herald about air quality in bars with smoking vs nonsmoking... The article cites EPA and DNR standards again, and while this isn't a new tactic to be deployed in Wisconsin, I finally put two and two together and figured out why this is wrong.</p>
<p>OSHA regulates workplaces. The EPA and DNR regulate outdoors. Why is this important? because OSHA is tasked with protecting worker's health and safety and does so in an objective manner. It's all about science where OSHA is concerned because OSHA recognizes that there is something called reality and if we bend reality to acomodate all of the goons out there, we would halt the wheels of this country.</p>
<p>The EPA and DNR, on the other hand, are run by hard-core zealots who will stop at nothing to eliminate pollution of any kind. They generally don't care about the overall well being of an economy or often times of the human race. It is rare that I see regulations stem from these organizations and think to myself "gee, that makes perfect sense!" In fact, it's usually the opposite as I roll my eyes and wonder why these people hate the human race so much.</p>
<p>So this all makes perfect sense. Recently both LaCrosse and Wausau have had studies done that cite back to air pollution stadards set forth by the WIDNR, which struck me as odd. After all, we're comparing apples to oranges here, yet it's cited as if they're one in the same. But now it makes sense.</p>
<p>The DNR's standards reflect a far lower threshold for what is considered "polllution" than OSHA does. To further that, the DNR does not differentiate between different forms of respirable suspended particles (RSP), which is incorrect in itself. If you go to OSHA and try and find a permissible exposure limit for RSP, you will find nothing. The indoor air quality section and the air contaminants sections breaks down each contaminant but gives no particular limit for RSP. So now we have to get into the actual composition of the smoke's RSP and how much of what is actually there. Good luck.</p>
<p>But then we go to the <a href="http://dnr.wi.gov/air/aq/pollutant/pm10.htm">WIDNR</a>, they lump all particles into a single catagory and set a limit based on that. There is no distinction between <em>types</em> of particles despite the fact that OSHA specifically states that different compositions have different effects (and by default then, different permissible exposure limits).</p>
<p>So why, then, are we now using the DNR's standards for workplace air quality? It's taking an orange and calling it an apple - and even though we know it's an orange we don't question it because the "experts" tell us so.</p>
<p>It's this sort of thing that is allowing the nannies to run roughshod over us.</p>

I happened to take care side by side with the primary nurse who took care of this 20 years old expectant mother. Reader discretion advised.</span></p>

<p class="MsoNormal" style="margin:0;"><span style="font-size:10pt;font-family:Verdana;">SYNOPSIS</span></p>
<p class="MsoNormal" style="margin:0;"><span style="font-size:10pt;font-family:Verdana;">This young mother had a history of malignant lymphoma and underwent successful treatment. She was visiting cancer specialists often and all scans and blood test showed she had no cancer. She was in cancer remissions for over 3 years and never took anything for granted. Here is the story.</span></p>
<p class="MsoNormal" style="margin:0;"><span style="font-size:10pt;font-family:Verdana;"> </span></p>
<p class="MsoNormal" style="margin:0;"><span style="font-size:10pt;font-family:Verdana;">The shift started as usual and assignments allocated by intensive care unit charge nurse. We all took assignments and one nurse was assigned to this 20 years old expectant mother. The fact that she was expectant, young and medically needy made her to be of higher acuity. This warranted 100% attention from top qualified registered nurse.</span></p>
<p class="MsoNormal" style="margin:0;"><span style="font-size:10pt;font-family:Verdana;"> </span></p>
<p class="MsoNormal" style="margin:0;"><span style="font-size:10pt;font-family:Verdana;">The 20 years old had been in cancer remission for over three years. She underwent serious testing before she became pregnant. Gynecologists and cancer specialists closely monitored her pregnancy every month. Her scan 2 months before she was admitted in ICU was clear and she was cancer free. </span></p>
<p class="MsoNormal" style="margin:0;"><span style="font-size:10pt;font-family:Verdana;"> </span></p>
<p class="MsoNormal" style="margin:0;"><span style="font-size:10pt;font-family:Verdana;">Few weeks after her previous negative scan, she started to develop a cough, and something that she assumed was normal. After all, who doesn’t develop a cough no and then, especially when pregnant? The cough persisted and this time, she started developing shortness of breath. She immediately called her gynecologist who advised her to go to emergency room immediately. The gynecologist followed up with her in the emergency room and a scan showed an egg size lymph node tumor in her chest area.</span></p>
<p class="MsoNormal" style="margin:0;"><span style="font-size:10pt;font-family:Verdana;"> </span></p>
<p class="MsoNormal" style="margin:0;"><span style="font-size:10pt;font-family:Verdana;">Radiological scans like x-rays are not recommended for pregnant mothers but when the mother life is on the line, it is better to take the risk than not at all. The severity of breathing difficulty was getting worse by the hour. She was taken for CT scan that revealed multiple lymph nodes swellings, including 2 more golf ball size around the neck region. These two had the potential to obstruct the airway, something that could make it a deadly medical emergency.</span></p>
<p class="MsoNormal" style="margin:0;"><span style="font-size:10pt;font-family:Verdana;"> </span></p>
<p class="MsoNormal" style="margin:0;"><span style="font-size:10pt;font-family:Verdana;">She was admitted to intensive care unit and after series of top qualified physicians and gynecologist, they decided to start her on radiation therapy the following day. These tumors were inoperable because her platelets level was critically low. She was admitted in ICU to receive intravenous platelets and stabilize her electrolytes levels in order to start radiation therapy and emergency surgery if needed. </span></p>
<p class="MsoNormal" style="margin:0;"><span style="font-size:10pt;font-family:Verdana;"> </span></p>
<p class="MsoNormal" style="margin:0;"><span style="font-size:10pt;font-family:Verdana;">All consent had been signed and it was just a matter of few hours before she could begin aggressive cancer treatment, this time, to save her life.</span></p>
<p class="MsoNormal" style="margin:0;"><span style="font-size:10pt;font-family:Verdana;">It is a common practice in intensive care unit to secure the airway by intubation to patient with poor oxygenation and those with a potential of losing their airway. Her case was one of those that required intubation. Several attempts to intubate her were unsuccessful because these cancer tumors were blocking the airway. However, she still could keep her oxygen saturation at 100% on 40% oxygen.</span></p>
<p class="MsoNormal" style="margin:0;"><span style="font-size:10pt;font-family:Verdana;"> </span></p>
<p class="MsoNormal" style="margin:0;"><span style="font-size:10pt;font-family:Verdana;">In ICU, the emergency anesthesia team was paged STAT to re-attempt the intubation as it was becoming harder by the minute for her to maintain oxygen saturation above 92%. This time, she was on 100% non-rebreather mask and still could not maintain good oxygen saturation. Her family was by bedside holding her hands as bags of platelets infused to bring up her platelet level. Without normal platelet levels, any attempt to operate could have made her to bleed to death.</span></p>
<p class="MsoNormal" style="margin:0;"><span style="font-size:10pt;font-family:Verdana;"> </span></p>
<p class="MsoNormal" style="margin:0;"><span style="font-size:10pt;font-family:Verdana;">When attempting to intubate, doctors discovered something even more serious. Those tumors had a pulse of their own, making them even harder to operate especially to a patient with low platelets level.</span></p>
<p class="MsoNormal" style="margin:0;"><span style="font-size:10pt;font-family:Verdana;"> </span></p>
<p class="MsoNormal" style="margin:0;"><span style="font-size:10pt;font-family:Verdana;">There was no way to save the 26 weeks baby in her uterus with platelet levels &#60;2,000. Her urine was pink, indicating spontaneous bleeding from her bladder and urethra. Her eyes were bleeding despite how fast platelets were transfused. Her cancer was growing by the minute. </span></p>
<p class="MsoNormal" style="margin:0;"><span style="font-size:10pt;font-family:Verdana;"> </span></p>
<p class="MsoNormal" style="margin:0;"><span style="font-size:10pt;font-family:Verdana;">She started gasping for the air, only a few hours after being admitted in the ICU. Every hospital in the state she had ever gone had recommended she go to a level #1 cancer and trauma hospital. She was in the right place for this condition.</span></p>
<p class="MsoNormal" style="margin:0;"><span style="font-size:10pt;font-family:Verdana;"> </span></p>
<p class="MsoNormal" style="margin:0;"><span style="font-size:10pt;font-family:Verdana;">2200: Oxygen saturation was in high 80’s, already lower than what is required to maintain acidity and alkalinity balance of the blood. An emergency meeting was held between the surgery, medical, oncology and gynecology teams. They came up with no answers. </span></p>
<p class="MsoNormal" style="margin:0;"><span style="font-size:10pt;font-family:Verdana;"> </span></p>
<p class="MsoNormal" style="margin:0;"><span style="font-size:10pt;background:yellow;font-family:Verdana;">2100: Breaking news: There is nothing medically possible that can be done at this time to save her life. The neck region tumors were swelling by the minute. That explained why it was becoming harder and harder for her to breathe by the minute.</span></p>
<p class="MsoNormal" style="margin:0;"><span style="font-size:10pt;font-family:Verdana;"> </span></p>
<p class="MsoNormal" style="margin:0;"><span style="font-size:10pt;font-family:Verdana;">2200: The fetal heart rate this time was becoming slower and slower. Again, there was nothing that could have been done that soon. The attempt to remove the baby would have been bleeding to death.</span></p>
<p class="MsoNormal" style="margin:0;"><span style="font-size:10pt;font-family:Verdana;"> </span></p>
<p class="MsoNormal" style="margin:0;"><span style="font-size:10pt;font-family:Verdana;">2300: <span style="background:yellow;">bleeding was evident even from the vagina</span>. Vaginal bleeding obviously indicated intrauterine bleeding. In a few minutes, we lost the fetal heart rate. Nothing could have been done. Everyone around could not avoid bursting in tears including us the nurses and doctors themselves.<span>  </span>The gynecologist pulled the family in a private room.</span></p>
<p class="MsoNormal" style="margin:0;"><span style="font-size:10pt;font-family:Verdana;"> </span></p>
<p class="MsoNormal" style="margin:0;"><span style="font-size:10pt;background:yellow;font-family:Verdana;">2400: Breaking news. We have lost the fetus! It was too late to do anything and the rate by which we were losing the ability to breathe was too fast to maintain fetal oxygenation. But the mother was still conscious and fighting for her dear life</span></p>
<p class="MsoNormal" style="margin:0;"><span style="font-size:10pt;font-family:Verdana;"> </span></p>
<p class="MsoNormal" style="margin:0;"><span style="font-size:10pt;font-family:Verdana;">0100: heart rate was in 150’s and blood pressure was trending downwards. Oxygen saturation was getting worse by the minute. Attempts to breathe were becoming harder by each breath. She could be heard gasping for breath from a distance.</span></p>
<p class="MsoNormal" style="margin:0;"><span style="font-size:10pt;font-family:Verdana;"> </span></p>
<p class="MsoNormal" style="margin:0;"><span style="font-size:10pt;background:yellow;font-family:Verdana;">0200: breaking news: She became unresponsive but the monitors could show heartbeats and a low blood pressure. The heart rate this time is coming down slowly. The heart is about to give up without oxygenation. The tumors had also blocked the access for emergency tracheotomy.</span></p>
<p class="MsoNormal" style="margin:0;"><span style="font-size:10pt;font-family:Verdana;"> </span></p>
<p class="MsoNormal" style="margin:0;"><span style="font-size:10pt;font-family:Verdana;">0300: The family signed does not resuscitate order (DNR) despite their denial and painful impeding and irreversible/unstoppable loss. </span></p>
<p class="MsoNormal" style="margin:0;"><span style="font-size:10pt;font-family:Verdana;"> </span></p>
<p class="MsoNormal" style="margin:0;"><span style="font-size:10pt;font-family:Verdana;">0330: No chest movement of breathing could be detected by visualization or by the monitor. It was just a matter of minutes before the heart could stop.</span></p>
<p class="MsoNormal" style="margin:0;"><span style="font-size:10pt;font-family:Verdana;"> </span></p>
<p class="MsoNormal" style="margin:0;"><span style="font-size:10pt;font-family:Verdana;">0350. The heart stopped with her whole family holding her hands. She was dead. The curtains were drawn to give the family some privacy and time to digest the loss.</span></p>
<p class="MsoNormal" style="margin:0;"><span style="font-size:10pt;font-family:Verdana;"> </span></p>
<p class="MsoNormal" style="margin:0;"><span style="font-size:10pt;font-family:Verdana;">We all tried our best to save her life. The obvious was impossible due to speed at which these events were happening and underlying conditions. <span style="background:yellow;">The 20-year-old American girl and a 26 weeks old fetus were dead. A life lost to cancer.</span></span></p>
<p class="MsoNormal" style="margin:0;"><span style="font-size:10pt;font-family:Verdana;"> </span></p>
<p class="MsoNormal" style="margin:0;"><span style="font-size:10pt;font-family:Verdana;">The most disturbing thing was, we helplessly watched her suffocate to death! Painful!</span></p>
<p class="MsoNormal" style="margin:0;"><span style="font-size:10pt;font-family:Verdana;"> </span></p>
<p class="MsoNormal" style="margin:0;"><span style="font-size:x-small;font-family:Verdana;">Update: I have had a lot of mixed reactions from the readers of this article and it is expected. It is a disturbing story. A lot of people are asking why the baby was not saved at a viable age of 26 weeks gestation. Others are stating that there is no way cancer can grow that first.</span></p>
<p class="MsoNormal" style="margin:0;"> </p>
<p class="MsoNormal" style="margin:0;"><span style="font-size:x-small;font-family:Verdana;">First about the baby. Medical procedures like those requires consents from the patient. Where the patient is not able to sign such consents, a legal next of kin can do it with witness. If Next of kin cannot be reached and the patient cannot sign the consents, two phycisians from different team can sign the concent, something you may hear being called "double doc".</span></p>
<p class="MsoNormal" style="margin:0;"> </p>

<div class="MsoNormal" style="margin:0;"><span style="font-size:x-small;font-family:Verdana;">The patient immediate family wanted the mother to be saved but there was no concent available for such procedures.</span></div>

<div class="MsoNormal" style="margin:0;"><span style="font-size:x-small;font-family:Verdana;">The hemodynamics for the mother were severely compromised and in such events, the fetal circulation from the mother shunts all resources to the vital organs to save the mother, hence, the reason why the baby died first.</span></div>

<div class="MsoNormal" style="margin:0;"><span style="font-size:x-small;font-family:Verdana;">On CT-scan, The baby was nodular (I forgot to mention that), a reason good enough not to attempt the surgery.</span></div>

<div class="MsoNormal" style="margin:0;"><span style="font-size:x-small;font-family:Verdana;">The risks of attempting a surgery were higher than not doing it!</span></div>

<content:encoded><![CDATA[<p>Desde que li uma reportagem no <em>The New York Times</em> sobre uma <strong>senhora alemã </strong>que pediu <strong>ajuda </strong>a um amigo para <strong>se suicidar</strong>, trazendo à tona a discussão sobre o <strong>"direito de morte",</strong> tenho pensado no assunto.</p>
<p>Calma, não estou depressiva, suicida, nem nada. Obviamente repudio assassinatos. Apenas parei pra <strong>refletir</strong> sobre o <strong>direito </strong>que cada um de nós possui de decidir sobre a <strong>própria existência. </strong></p>
<p>A <strong>eutanásia</strong>, proibida no Brasil, não seria uma maneira <strong>digna </strong>de acabar com o sofrimento de alguém que não suporta <strong>a dor e a agonia </strong>de sua respiração? O pedido de alguém <strong>para não ser ressucitado</strong> em casos gravíssimos, deveria ser levado em consideração? E o caso da<strong> senhora alemã, infeliz, </strong>que preferiu terminar sua vida a ser colocada num asilo, merece ser discutido?</p>
<p>Pesquisando na internet, encontrei alguns textos espíritas e muitos textos legais tratando do <strong>direito de morrer</strong>. Um amigo sugeriu a leitura do "Suicídio do Jovem Werther", o qual ainda não tive a oportunidade de ler.</p>
<p>Se alguém aí quiser discutir esse assunto, vamos lá. Se não, pelo menos dividi com vocês algo que estive pensando nas últimas semanas...</p>

<p>The <a class="zem_slink" title="Iowa Department of Natural Resources" rel="wikipedia" href="http://en.wikipedia.org/wiki/Iowa_Department_of_Natural_Resources">Iowa Department of Natural Resources</a> will be releasing the weekly fishing report on <strong>Thursdays</strong> in an effort to provide the latest information heading into the weekend. The weekly fishing report is compiled from information gathered from local bait shops, angler creel surveys, and county and state parks staff.</p>
<p>For current information, contact the district fisheries office. Phone numbers are listed with each district report.</p>
<p><strong>FLOODING IMPACTS ON FISH</strong></p>
<p>The unprecedented volume of water in many areas during the flood will minimize the impact from any chemicals, waste or other toxins mixed in with the water. Nothing in the water is going to hurt the quality or the edibility of the fish. These fish are safe to eat.</p>
<p>In general, more water means more fish. Growth of fish is great in expanded habitats when lake and stream water levels are high. It is expected the flood conditions in many streams will have an adverse impact on <a class="zem_slink" title="Smallmouth bass" rel="wikipedia" href="http://en.wikipedia.org/wiki/Smallmouth_bass">smallmouth bass</a> reproduction this year. The floods will rearrange the habitat in streams with some deep holes filling in while others are created.</p>
<p>A larger issue is that of sediment washing into streams, lakes and ponds, covering and reducing habitat and causing <a class="zem_slink" title="Water quality" rel="wikipedia" href="http://en.wikipedia.org/wiki/Water_quality">water quality</a> problems.</p>

<content:encoded><![CDATA[<p>The good people at <a href="www.southwickclothing.com" target="_blank">Southwick</a> sent along the spring summer 2009 catalog. There has been a buzz around the storied American brand with the collaboration with <a href="http://acontinuouslean.com/2008/07/02/wrinkled-and-cool-engineered-garments-ss09/" target="_self">Engineered Garments</a> and more recently the company's acquisition by <a href="http://www.dnrnews.com/site/article.php?id=2139" target="_blank">Brooks Brothers</a>. Plans for Southwick were further discussed in <a href="http://www.dnrnews.com/site/article.php?id=2173" target="_blank">DNR today</a>. "In an interview, Claudio Del Vecchio, CEO of Brooks, said Southwick is a “major supplier” to Brooks Brothers, 'so we were very interested in them and seeing them improve their quality.'" I think Southwick being sold to Brooks Brothers isn't necessarily a bad thing. Like others, I hope that Brooks will do the right thing and keep Southwick pure.</p>

<p style="text-align:justify;">According to <a href="http://www.dnrnews.com/site/article.php?id=2142" target="_blank">DNR</a>, "<em>Shot on location in Miami by renowned photographer Bruce Weber, who shot Calvin Klein Underwear’s first ad campaign 25 years ago, the sexy black-and-white images epitomize the very clean yet sensual aesthetic that Calvin Klein has become so well known for. Neff, who is the face of the company’s fragrance, Calvin Klein Man, was featured alongside Kate Bosworth in the spring ad campaign for Calvin Klein Jeans. He succeeds Academy Award–nominated actor Djimon Hounsou as Calvin Klein’s latest body.</em>"</p>

<p>Anna Wintour stopped by Men's Fashion Week in Milan, in support of Men's Vogue and DNR got to do a micro-mini Q&#38;A with her, the full Q&#38;A will be in Monday's DNR. When asked about how Men's Fashion Week compared to women's,<!--more--> Ms. Wintour stated "It's much smaller in scale, which makes it very charming. It’s easier to see people and just to get around." For now at least, I bet Men's fashion is gonna grow and be as big as the ladies, or bigger! <p>Kažkur septintą valandą važiavau su dviračiu iš MRU treniruoklių salės. Važiavau lėtai, po treniruotės šiek tiek pavargau, be to, šios savaitės pabaiga buvo nelabai lengva. Taigi, važiuoju sau ramiai netoli Rygos g. ir Buivydiškių g. sankryžos ir jau prie "Širdies chirurgijos centro" nutariau pasukti į dešinę ir per mašinų stovėjimo aikštelę sutrumpinti sau kelią. Bet nepatikėsite, tikrai ne mašina į mane atsitrenkė, netgi ne stulpas pasimaišė, o kažkoks dviratininkas, lekiantis dideliu greičiu. Didelių traumų negavau nei aš, nei mano dviratis, na, man šiek tiek kliuvo į alkūnę, o dviračiui šonu pasisuko vairas. Į mane atsitrenkė iš galo. Vaikinui, kuris atsitrenkė, berods sulūžo priekinis žibintas, nes tiesiog iššoko iš vairo, ir pats jis vos nenukrito.</p>
<p>Iš karto paklausiau vaikino ar jam viskas gerai, ar nieko nenutiko dviračiui, tiesiog parodžiau susirūpinimą. Kartu pasijuokėme, pasiaiškinome, kas buvo šiuo atveju neteisus, ir nuvažiavome savo keliais.</p>
<p>Taigi, jei priešais save, važiuodami dviračiu, matote kitą dviratininką, lenkite jį atsargiai, nes ne visi turi šoninius veidrodėlius, o atgal pažiūrėti ne taip jau ir paprasta.</p>

<content:encoded><![CDATA[<p>Today was a rewarding day.  I was called by a primary care team to assist with the care of a 25 year-old girl who is dying of cancer.  This was a situation that, as unfortunate as it was, had been made more complicated by growing distrust on the part of the family.  As the resident told me her story, he mentioned that the patient was being seen by the Palliative Care Service.  This was unusual, since the same pain specialist oversees both the Palliative Care Service and the Chronic Pain Service.</p>
<p>I agreed to come, not as a formal pain consultant (since my boss was already technically on the case), but as someone that might visit with the family and offer another perspective.  This was mainly a psycho-social consultation.  I did meet briefly with the patient, but spent most of my time in the family room with the father, the rabbi, and the primary care physician.  When I arrived, they were on the verge of signing Do Not Resuscitate/Do Not Intubate orders, and the father wanted to make sure that in spite of signing the paperwork, the family would still have a voice in the medical care.</p>
<p>This, of course, was easy.  I assured him that these measures simply meant we would not take aggressive measures to unnaturally prolong his daughter's life.    They absolutely would not change the quality or quantity of their interaction with their physicians.</p>
<p>I spent the next thirty minutes asking about their goals and expectations, listening to their fears and frustrations, and assuring them that we would in no way abandon them in the face of the daughter's rapidly deteriorating health.  I say "them" because in this case, my role of physician seemed to explicitly extend to include the patient, her parents, and the rabbi.  We clearly definited several goals: comfort &#38; pain control, avoiding sedation, tolerating oral medications, being able to go home.</p>
<p>Tangibly, I was rewarded by being able to accomplish what the primary team and the regular Palliative Care Team had been unable to do, that is, to adjust the pain regimen that might facilitate comfort and be a step closer to home.  I attribute this not to any special skill, but to being
